Referral information will be provided and funding options identified. We have a particular specialization in working with older adults who have had changes in mood or struggle with a dementia such as Alzheimer's disease.
We start with a professional assessment of an older adult's needs and develop an individualized Care Recommendation Plan. Our consultation process allows families and older adults to choose among a range of carefully selected services. Comprehensive Evaluation of medical, emotional, or memory problems that are interfering with daily functioning, impacting quality of life. Family education and counseling: information regarding the older adult's needs and care options Family mediation: communication around difficult issues, as well as developing a vision and common ground around care needs. Coaching for family and caregivers to best meet the needs of the older adult in their care. Education about home care and community living options: hiring help in the home, assisted living and independent living communities, 55 plus housing, dedicated memory care residences, and nursing homes. Recommend when to seek legal services, financial planning, psychiatric and medical referrals, and other services. Identify options related to planning for end of life care such as Hospice and Palliative Care. Inform about Family and Medical Leave Act, Power of Attorney verses Conservator, VA Benefits, options regarding medical insurance decisions. Provide guidance in response to suspected elder abuse or exploitation. Advise on driving: how to respond to changes in driving ability. Prepare a disaster plan for the care of older adults in case of emergencies such as a storm, power failure, flooding or dislocation.

Clients meet with an Elderpath consultant, a professional in geriatrics, to discuss resources and plan for the future. The ElderPath consultant provides extensive expertise in geriatric care particularly when there are memory or mood changes. The end result is a Care Recommendation Plan that supports your goals.
Family Mediation Services
ElderPath offers family mediation services when family members are at a crossroads regarding the care of an older adult. The mediator works with family members to come to a consensus around care needs.
Evening/Weekend availability
For your convenience, an ElderPath consultant is available evenings and weekends. The consultation can take place in your home or our Glastonbury office.
ElderPath is associated with Dr. Harry Morgan and The Center for Geriatric and Family Psychiatry in Glastonbury. We offer direct referral to this Center.
Consultants are sensitive to the needs of older adults in diverse cultures and in the LGBT community.
